Accommodations subject to advance booking and availability of dates. Must mention gift certificate when booking and provide original at check-in. Completion of a reservation form is required. A refundable damage deposit of US$350 is required at check-in. No cash value. Must be redeemed by November 1, 2010. Airline certificates valid for travel by the passenger exclusively on scheduled United, United Express and Ted operated flight and is not valid for travel on flights operated by other airlines (such as United-marked code share and Star Alliance flights.) Valid for roundtrip travel from the 50 United States, Puerto Rico, U.S. Virgin Islands and Atlantic destinations served by United. Stopovers, open jaws or circle trips are not permitted. Valid travel dates of 11/14/09 - 11/13/10. Travel must be booked at least 7 days in advance of departure. Travel must include a Saturday night stay. The maximum stay is 30 days. No blackout dates. Travel will be booked in S or T class in Economy Class. Seats are limited and may not be available on all flights or on all days of the week when you travel. Certificates are not transferable and may not be bartered or sold. Passenger traveling on this free ticket may not accrue Mileage Plus miles or earn miles toward Mileage Plus elite status. This ticket is not upgradeable. Ticket-by-mail is allowed (where available.) All tickets will be issued electronically unless customer requests otherwise (paper ticket fees will apply.) Ticketing must occur within 24 hours of making a reservation, whichever sooner. Certificate may not be used with tickets purchased via the internet. When using ticket-by-mail, please allow 14 days processing time. The passenger is responsible for all taxes/fees including the September 11th Security Fee of up to $10 maximum per roundtrip, Passenger Facility Charges of up to $18 and a $3.30 per flight segment tax. A flight segment is defined as one takeoff and one landing. For travel from Alaska to Hawaii, passengers will pay a $7.30 (each way) international departure tax. For travel from Puerto Rico and the U.S. Virgin Islands, passengers will pay a $14.50 (each way) international departure tax. Additionally, for travel from Canada, passengers are responsible for U.S. Inspection Fees of $7 and for all taxes and fees imposed by the Canadian government of up to $19.50 per roundtrip and a $29 per roundtrip international departure and arrival tax if outside of the 225 mile buffer zone. Additional restrictions may apply. Certificate is required for discount and must be presented at the time of ticketing. Accept original certificate only. Not extendible, not combinable with other air travel certificates or discount fare offerings (Mileage Plus awards, Silver Wins Plus Awards, companion, meeting, group, tour, senior citizen, student, child, travel package, travel industry discount, military, government, joint, interline, wholesale, bulk, prepaid tickets and miscellaneous charge orders.) Not replaceable if lost or stolen. Protection for flight irregularities will be on United/United Express/Ted flights only. Certificate applies to new purchases only. Certificate may not be used in conjunction with any wholly or partially unused ticket, or with any previously issued non-refundable ticket. Certificate has no cash value and is non-refundable by ULS or United. Changes to date/time are allowed for service charge of $100. No charges to origin or destination are permitted.
